I’m Kelli Kelderhouse, a certified Family Nurse Practitioner with 30 years of experience in healthcare and nursing. For the past 8 years, my career and passion have shifted to wellness, with a focus on helping patients through challenging hormonal transitions and providing bio-identical hormone replacement therapy, or BHRT.
At RWS in Hampstead, NC, I specialize in menopause care, bioidentical hormone replacement therapy, medically supervised weight loss and management, and revitalizing therapeutic IV fluid treatments. My approach blends up-to-date and cutting-edge medical treatments with an emphasis on proactive wellness. Finally, and most importantly, I want to provide a comfortable and intimate setting where you feel seen and heard while I serve as a provider, partner, and cheerleader in your individual health and wellness journey.

You have options! You may choose to remain with your previous practice and transition to another provider there. Alternatively, I’m available to provide care at Revamped Wellness Solutions on an interim basis. If preferred, I can also assist you in finding a new provider. Please reach out, and we’ll help transfer your records to ensure seamless care.
1. Contact your previous/other provider’s office and request a records release form. 2. Complete the form, specifying that records should be sent to Revamped Wellness Solutions. 3. Provide our contact information: Kelli Kelderhouse FNP-C, Revamped Wellness Solutions PLLC, Fax: 1-833-740-3618 . 4. Allow time for processing (typically 7-14 business days).
Alternatively, you can also fill out a Release of Information consent form here at Revamped Wellness Solutions that we can send to your previous/other provider office. This form can be emailed to you to sign and return or can be filled out and signed during your next visit.

At Revamped Wellness Solutions, we prioritize an individualized experience, which requires us to forgo direct billing with insurance companies due to the extensive administrative demands. However, we’re happy to provide a superbill that you can submit to your insurance carrier for potential reimbursement. Please note that many of our services may not be covered by standard insurance policies. If you have questions or would like to receive a superbill, don’t hesitate to contact us for further assistance.
